Cedar-apple rust is treated by spraying both the apple (or crabapple) and the juniper or cedar host with a fungicide labeled for rust during an active outbreak, repeating that spray 14 to 21 days later, then following up the next spring with a three-spray preventive schedule timed to apple bloom. Spraying only one of the two hosts leaves the fungus free to complete the other half of its lifecycle and reinfect the tree the following year.
Cedar-apple rust is a fungus that cannot complete its life cycle on a single species, it alternates between an apple tree and a cedar or juniper tree to reproduce. Spores from brown and red galls on a juniper or red cedar blow onto apple leaves in spring, and later in the season spores produced on the infected apple tree blow back onto juniper and red cedar trees, starting a new infection there. Cedar-apple rust is closely related to cedar-hawthorn rust and cedar-quince rust, which follow the same two-host pattern using hawthorn or quince in place of apple.
Small, pale yellow spots appear on the upper surfaces of apple leaves in mid to late spring, the earliest visible sign of infection. As the disease progresses, small black or brown spots develop in the center of those larger yellow spots, and heavily infected leaves and fruit can drop prematurely. Both fruit-producing and ornamental flowering apple varieties are affected.
The infection shows up as brown, kidney-bean-shaped galls on the branches, which swell and produce bright orange, gelatinous spore horns during wet spring weather, the stage that releases spores toward nearby apple trees. Those galls are the source of the infection on the juniper side, even though the visible leaf damage described above shows up on the apple host instead.
Spraying every infected and uninfected cedar, juniper, and apple tree in the area is the standard approach, since treating only the trees already showing symptoms leaves the untreated hosts free to keep the cycle going. A fungicide labeled for rust, such as Fungi Max Multi-Purpose Fungicide, whose active ingredient myclobutanil is specifically listed as controlling rust, fits that first round of spraying, repeated a second time in about 14 to 21 days.
A three-spray preventive schedule the next spring keeps a new infection from taking hold: the first spray at flowering, the second when about 75% of the flower petals have dropped, and the third about 10 to 14 days after that. This schedule targets the window when apple blossoms and new leaf tissue are most exposed to spores released from juniper galls.
No. Because the fungus needs both hosts to complete its lifecycle, clearing symptoms from one side without treating the other only interrupts half the cycle, and the untreated host can reintroduce the infection the next season. Spraying both the apple and the juniper or cedar host in the same round is what actually breaks the cycle rather than just suppressing visible symptoms.
Related but distinct. Cedar-apple, cedar-hawthorn, and cedar-quince rust are separate fungi that share the same two-host pattern, alternating between a juniper species and either apple, flowering crabapple, hawthorn, or quince depending on which rust is involved.
Not reliably. Rust spores travel long distances on wind, so a juniper on a neighboring property can still be the source of an infection even after every juniper nearby has been removed.

For a tree being replaced or newly planted, yes. Choosing a cultivar bred for rust resistance reduces how much spraying an apple tree needs going forward, though an already established, susceptible tree still needs the spray schedule above to manage an active infection.
